PARTICIPANT WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK,
INDEMNIFICATION AGREEMENT & ACKNOWLEDGMENT OF MAT RULES**
This legally binding Agreement is entered into between SPBJJ LLC d/b/a Shark Pit Jiu Jitsu (“Shark Pit Jiu Jitsu” or “Released Parties”) and:

By signing this Agreement, {name} (“Participant”) acknowledges, understands, and agrees to the following:
1. ASSUMPTION OF RISK
Participant understands and accepts that Brazilian Jiu Jitsu, grappling, martial arts, and physical fitness training involve inherent risks, including but not limited to sprains, strains, joint injuries, skin infections, concussions, serious bodily harm, permanent disability, paralysis, or death. Participant knowingly and voluntarily assumes all such risks, whether known or unknown, even if arising from negligence.
2. RELEASE OF LIABILITY & HOLD HARMLESS
Participant voluntarily releases, waives, and forever discharges Shark Pit Jiu Jitsu, SPBJJ LLC, its owners, instructors, employees, contractors, volunteers, members, guests, and all individuals or entities connected to the gym from any and all liability or claims for injury, damage, loss, medical expenses, or death arising from participation, presence in the facility, or travel to/from the gym — whether caused by negligence or otherwise.
3. PHYSICAL CONDITION, FITNESS & MEDICAL TREATMENT
Participant affirms they are in good physical condition and capable of safely engaging in training. Participant understands:

Only basic first aid may be provided.

They have been advised to obtain a physician’s clearance before beginning training.

Training may result in abnormal blood pressure, fainting, injury, heart complications, or death.
Participant may stop training at any time if they feel ill, lightheaded, or injured.
4. MEDIA RELEASE
Participant grants Shark Pit Jiu Jitsu the unrestricted right to use photographs, audio, or video recordings of them for advertising, promotion, publications, social media, and any lawful purpose, without additional consent or compensation.
5. RULES, DIRECTION & CONDUCT
Participant agrees to follow all instructor directions, safety procedures, and Mat Rules (included below). Failure to comply may result in removal from class or termination of membership without refund.
6. INDEMNIFICATION
Participant agrees to indemnify and hold harmless the Released Parties from any claims by third parties arising from Participant’s actions, negligence, or violation of gym rules.
7. CERTIFICATION OF CAPACITY & AGE
By signing below, Participant certifies they:

Are not under the influence of drugs or alcohol.

Are not suffering from illness or condition that would impair safe participation.

Are 18 years or older, or a parent/guardian will sign on their behalf.
MAT RULES
By signing below, Participant acknowledges reading, understanding, and agreeing to abide by the following Mat Rules:

Safety is our first priority. Always be careful and controlled with your training partners. Treat people as you would like to be treated.

Toenails and fingernails must be kept trimmed. Proper hygiene must be maintained. All gis and training clothing must be washed after every class.

Remove any jewelry, watches, piercings, and anything that could catch or tear skin. Long hair must be secured.

A tap can be signaled by tapping your opponent or the mat, or saying “tap.” When your partner taps, immediately stop all pressure.

Tap early and tap often. Do not apply submissions too quickly or forcefully.

No striking of any kind.

No slamming. If you lift your partner, you are responsible for safely returning them to the mat.

No direct throat grabbing, or eye gouging.
-
No small joint manipulation. You must grab four or more fingers or toes at a time.

Groin protection and mouth guards are recommended but not required.

No shoes or bags on the mats. No bare feet between the bathroom/locker room and the mats.

Remain silent and attentive while instructors are teaching. Phones must be stowed.
-
Parents may observe from the sidelines only and may not coach.
-
Do not train while sick. No training with any skin infection.
-
Pick up after yourself.
-
Do your best to arrive on time. If late, wait at the mat edge for instructor permission to enter.
